A bag machine operator was retrieving a bag that jammed the machine and severed the left index fingertip. The bypass switch on the machine was activated. Once the switch is activated, the machine cycles one more time before it hits the home position and stops. The door was opened before the cycle completed and the star wheel pinched the tip of her left index finger, severing it just above the first knuckle. The incident occurred October 7, 2015 at approximately 9:35hrs. |
